Registration Forms
If you would like to register your child with us please complete a registration form by either, downloading a copy here and emailing to us at primetimekidsclub@tiscali.co.uk or posting it to us at Prime Time Kids Club, C/o Wareham St Mary Primary School, Streche Road, Wareham, Dorset BH20 4PG. We also value people coming to visit us with their children, so please feel free to call us on 07715103021 to arrange a visit, where we can provide you with a copy of the registration forms.
When completing registration forms you must disclose all the information asked for and keep us updated if things change. If it comes to light we have not been given all the information asked for we will remove your place. Prime Time Kids Club works with other professionals to ensure individual children’s needs are being met. It is particularly important if your child has any additional needs we are informed with plenty of time to be able to consult with yourselves as parents and other professionals to ensure we are the best place for your child and able to cater for your child adequately.
